4. State of nationality and State of establishment- (real and effective industrial or commercial establishment)
5. If legal entity, legal nature of entity ( corporation, firm, partnership, limited company).
6. The country and the unit within the country under which the laws of the legal entity is governed.
7. If applicant is claiming priority, the Country/Office and date of first filing, application number of first filing. The certified copy of application to be provided within three months of filing date of the present application.
8. Reproduction of mark 8cm x 8cm (Not required if mark consists solely of words)
9. Whether colour is claimed a distinctive feature of a mark.
10. Whether the mark is three dimensional
11. Transliteration and translation of mark if mark is not an English word.
